- Insurance information and card
- Information about treatments and medications
- Medical records

It is important to fill out your history completely. Our team of professionals reviews this information with you to determine the best course of treatment. All of our treatment plans are co-developed with your input. Just print them out and bring them with you. All information is strictly confidential.
